When you hear the term "TBI" you immediately
expect the worst and as a minimum you expect it to be a significant
life changing injury. However there are many different levels of TBI
caused by many different types of accident.
The injury is wide ranging and may be as minor as brief loss of
consciousness for 15 minutes or less with a period of post-traumatic
amnesia of less than 1 hour or as catastrophic as persistent
vegetative state, where the injured person is unable to speak and does
not follow commands or make any response that is meaningful.
Typically the majority of
these cases are caused by high impact road traffic accidents and are
known as acceleration/deceleration injury. This is a closed head
injury often sustained in car accidents, where the brain moves
forwards and then backwards, rebounding against the walls of the
skull, causing damage to both the frontal lobes and the back of the
brain. It is usual for magnetic resonance imaging (MRI) scans to be
used for diagnosis as they enable detailed pictures of the brain to be
acquired using a scanning machine.
Other types of this type of
injury include
Post-concussion syndrome: a group of symptoms occurring after mild
head trauma that may persist for days, weeks or months
Post-traumatic amnesia (PTA): the period after being unconscious when
there may be confused behaviour and no continuous memory of day to day
events
Amnesia: failure of memory. Hence, post-traumatic amnesia (PTA) which
is the lapse of time until continuous recall occurs
Retrograde amnesia: the loss of memory of events for a period prior to
the trauma
In order to establish the extent of the damage and the effect on an
injured person in their daily life, a medical report from a consultant
psychologist or consultant psychiatrist is usually obtained.
A Consultant psychologist is a professional specialising in
counselling, including adjustment to disability. Psychologists use
tests to identify personality and cognitive functioning. This
information is shared with team members to assure consistency in
approaches. The psychologist may provide individual or group
psychotherapy for the purpose of cognitive retraining, management of
behaviour and the development of coping skills by the patient/client
and members of the family.
Consultant psychiatrists are able to provide the same assistance
although they tend not to be as “hands on” as a consultant
psychologist, however they can prescribe drugs for patients whereas a
psychologist cannot. Psychiatrists report on post-traumatic stress
disorder psychological and psychiatric effects of trauma.
Glossary of Terms
Open head injury:
an injury where there is penetration of the scalp and skull through to
brain tissue.
Head injury - moderate:
defined as being a condition where the patient has been in a coma for
6 hours, and a period of post-traumatic amnesia of up to 24 hours.
Head injury - severe:
defined as being a condition where the patient has been in a coma for
6 hours or more, or a post-traumatic amnesia of 24 hours or more.
Glasgow coma scale:
a score given to head injured patients starting immediately after the
head injury to measure the degree of unconsciousness. A score of 7 or
less indicates that the person is in a coma. A maximum score of 15
indicates that the person can speak coherently, obey commands to move,
and can spontaneously open their eyes.
Closed head injury:
damage to the brain where there is no penetration from the scalp or
skull through to brain tissue. Often there is no injury to scalp or
skull.
